Services

A coordinated interdisciplinary team of highly skilled and dedicated people work from one plan of care to provide medical and support services to patients and families. Our program includes:

Physican Services:

Hospice care is provided under the direction of a physician known as the "Medical Director". The medical director gives direction to the patient's team and consults with the patient's attending physician regarding the patient's care.

Nursing Services:

The physician-directed care is coordinated by a registered nurse. The medical director and registered nurses work closely with the patient's physician to ensure that the patient is kept as physically comfortable as possible. Considerable focus is given to pain control and symptom management. The hospice nurse makes scheduled patient visits, and provides instructions on patient care to family members and caregivers. In addition, nursing staff are on call 24 hours a day, seven days a week for emergency situations.

Home Health Aide Services:

St. Anthony's Hospice health care aides assist the family with bathing the patient and other personal care needs. Home health aides' schedules are adapted to meet the patient's and family's requirements. The aides also help with light household duties related to the care of the patient.

Social Services:

St. Anthony's Hospice clinical social workers provide assistance with financial concerns as well as emotional support, counseling and bereavement follow-up. They provide individual and family counseling to help the patient and family adjust to a terminal illness and loss. Our social workers are trained to work with both children and adult family members to help them understand and deal with loss. During visits, they offer emotional support, discuss the care the patient is receiving and answer any questions a loved one may have. They evaluate the support services needed by the patient and the family and facilitate communication between the family and other community agencies for support. If there are any issues that need to be resolved, the social worker will evaluate them and help find solutions.

Pastoral Care:

If requested,a hospice chaplain will make visits to offer spiritual support and prayer on behalf of the patient. Chaplain services also extend to the families and friends of hospice patients. Hospice chaplains bring compassion and support during difficult times. They do not take the place of the patient's own minister, but complement the existing spiritual support. If desired, a chaplain is available to perform sacraments and to conduct funerals or memorial services.

Bereavement Care:

Bereavement support for family members preparing for death,as well as follow-up support and counseling after the death, is provided through our hospice program.

Volunteer Services:

Hospice volunteers are available to help in many ways. If a family desires the services of a volunteer, special care is taken to match the appropriate volunteer with the patient and his/her family. Volunteers can provide caregiver relief, offer support or transport the patient to a medical appointment. They may pick up and deliver medication from a pharmacy. Or, they simply may visit and be a friend to the patient. Visit our volunteer section of this site.
